If you would like a copy of your medical records, we are happy to provide them to you.

If you are a member of our secured patient portal, you may sign on and choose “Request Medical Records”  This will allow you to request your medical records via a secured email.  Please remember to include exactly what medical records you are requesting (i.e., clinical notes, operative report,  entire chart, etc).  Your records will be emailed back to your secured patient portal.

If you do not use our web portal to request your records, you will need to sign a release authorizing us to send your records (Allina Health Release of Information Form).  Under privacy and security regulations, we are only allowed to provide the information that was generated by our practice.  We are not authorized to release any diagnostic, hospital data, referring physician notes, or any other information that was provided from an external source.
